• weight > or = 400 lbs.
• diabetes (type 2 or type 1)
• hypertension
• heart disease
• kidney disease
• liver disease
• active cancer
• post solid organ or bone marrow transplant
• HIV/AIDS
• pregnancy
• breastfeeding
• current smoker
• current recreational drug use
