• referred to SLU cardiac rehab facility

You may not qualify if…

• does not own app compatible smart phone
• dementia
• syncope
• valvular disease
• life expectancy < 1 year
• non-English speaking
• unstable angina
• decompensated congestive heart failure
• ventricular arrhythmia
