Age: 21 and older, up to 89. Sex: any. Healthy volunteers: not accepted.
You may qualify if…
- Age 21 years old or over
- Type 2 Diabetes Mellitus diagnosed for a minimum of 12 months
- Able to read in English or Spanish
- Plan to remain in the practice during the study period
You may not qualify if…
- Developmentally disabled
- Decisionally challenged
- Pregnant women
