You may qualify if…

• VA ECMO (all cannulation sites)
• Primary Cardiac Failure
• Septic Shock

You may not qualify if…

• VV ECMO
• Primary Respiratory Failure
• Congenital Diaphragmatic Hernia
• Persistent pulmonary hypertension of the newborn
• greater than 1 source of systemic perfusion
