You may qualify if…

• Symptomatic heart failure
• Left ventricular ejection fraction <= 35%
• QRS width >= 120 ms
• Indicated for an implantable cardioverter defibrillator

You may not qualify if…

• Indicated for a pacemaker
• Atrial tachyarrhythmias
