• Diabetes mellitus, type 2
• Obesity
• Ability to attend office visits in Manassas, Virginia

You may not qualify if…

• Dementia
• Institutionalized living
• Retinopathy, neuropathy
• Pregnancy
• Serious health problems such as heart disease or stroke
